ABSTRACT

Conhecer as representações sociais de profissionais de enfermagem que atuaram no atendimento aos pacientes acometidos pela COVID-19 diante da sobrecarga de trabalho. Método: Estudo de natureza descritiva com abordagem qualitativa utilizando como referencial a Teoria das Representações Sociais. O cenário foi um hospital que atendeu pacientes acometidos pela COVID-19. Os participantes foram profissionais de enfermagem. Os dados se deram através de entrevistas semiestruturadas. A análise ocorreu conforme a perspectiva de Bardin. Resultados: As representações retrataram condições de trabalho identificando desgaste mental diante em um cenário de inúmeras incertezas, de modo a suprir as necessidades exigida pela crise sanitária, impactando diretamente na sua saúde mental. Conclusão: Possibilidade da construção de um modelo de assistência que reestruture no processo de trabalho da enfermagem, minimizando possíveis sobrecarga de trabalho associados à sua saúde mental, otimizando assim a melhoria das suas condições de trabalho, fortalecendo respostas rápidas e precisas quando necessárias.(AU)


Objective: To learn about the social representations of nursing professionals who worked in the care of patients affected by COVID-19 in the face of work overload. Method: A descriptive study with a qualitative approach, using the Theory of Social Representations as a reference. The setting was a hospital that treated patients affected by COVID-19. The participants were nursing professionals. The data was collected through semi-structured interviews. The analysis took place according to Bardin's perspective. Results: The representations portrayed working conditions identifying mental exhaustion faced in a scenario of countless uncertainties, in order to meet the needs demanded by the health crisis, directly impacting on their mental health. Conclusion: The possibility of building a care model that restructures the nursing work process, minimizing possible work overload associated with their mental health, thus optimizing the improvement of their working conditions, strengthening quick and precise responses when necessary.(AU)


Objetivo: Conocer las representaciones sociales de los profesionales de enfermería que han trabajado en el cuidado de pacientes afectados por COVID-19 ante la sobrecarga de trabajo. Método: Estudio descriptivo con enfoque cualitativo, tomando como referencia la Teoría de las Representaciones Sociales. El escenario fue un hospital que atendía a pacientes afectados por COVID-19. Los participantes fueron profesionales de enfermería. Los datos se recogieron mediante entrevistas semiestructuradas. El análisis se realizó según la perspectiva de Bardin. Resultados: Las representaciones retrataron condiciones de trabajo que identifican agotamiento mental enfrentado en un escenario de innumerables incertidumbres, para atender a las necesidades demandadas por la crisis de salud, impactando directamente en su salud mental. Conclusión: Es posible construir un modelo de atención que reestructure el proceso de trabajo de enfermería, minimizando la posible sobrecarga de trabajo asociada a su salud mental, optimizando así la mejora de sus condiciones de trabajo, fortaleciendo respuestas rápidas y precisas cuando sea necesario.(AU)

ABSTRACT

A síndrome respiratória aguda grave (SRAG) é caracterizada por sintomas de febre alta, tosse e dispneia, e, na maioria dos casos, relacionada a uma quantidade reduzida de agentes infecciosos. O objetivo foi avaliar a prevalência dos vírus respiratórios Influenza A (FluA), vírus sincicial respiratório (RSV) e do novo coronavírus (SARS-CoV-2) em pacientes com internação hospitalar por SRAG. Estudo transversal, com pacientes em internação hospitalar com SRAG entre novembro de 2021 e maio de 2022. Dados sociodemográficos e clínicos e amostras da nasofaringe foram coletados/as, as quais foram submetidas à extração de RNA e testadas quanto à positividade para Influenza A, RSV e SARS-CoV-2 por meio da técnica de PCR em tempo real pelo método SYBR Green. Foram incluídos 42 pacientes, sendo 59,5% do sexo feminino, 57,1% idosos, 54,8% com ensino fundamental. A maior parte dos pacientes reportou hábito tabagista prévio ou atual (54,8%), não etilista (73,8%) e 83,3% deles apresentavam alguma comorbidade, sendo hipertensão arterial sistêmica e diabetes mellitus tipo 2 as mais prevalentes. Um total de 10,5% dos pacientes testou positivo para FluA, nenhuma amostra positiva para RSV e 76,3% positivos para SARS-CoV-2. Na população estudada, SRAG com agravo hospitalar foi observado em maior proporção, em mulheres, idosos e pessoas com comorbidades, embora sem significância estatística, sendo o novo coronavírus o agente etiológico mais relacionado, o que evidencia a patogenicidade desse agente e suas consequências ainda são evidentes após quase 2 anos de período pandêmico.


Severe acute respiratory syndrome (SARS) is characterized by symptoms of high fever, cough and dyspnea, and is in most cases related to a reduced amount of infectious agents. The objective was to assess the prevalence of respiratory viruses Influenza A (FluA), respiratory syncytial virus (RSV) and the new coronavirus (SARS-CoV-2) in patients hospitalized for SARS. Cross-sectional study, with patients hospitalized with SARS between November 2021 and May 2022. Sociodemographic and clinical data and nasopharyngeal samples were collected, which were subjected to RNA extraction and tested for positivity for Influenza A, RSV and SARS-CoV-2 using the real-time PCR technique using the SYBR Green method. 42 patients were included, 59.5% female, 57.1% elderly, 54.8% with primary education. Most patients reported previous or current smoking habits (54.8%), non-drinkers (73.8) and 83.3% of them had some comorbidity, with systemic arterial hypertension and type 2 diabetes mellitus being the most prevalent. A total of 10.5% of patients tested positive for FluA, no samples positive for RSV, and 76.3% positive for SARS-CoV-2. In the studied population, SARS with hospital injury was observed more frequently in women and the elderly, with associated comorbidities, with the new coronavirus being the most related etiological agent, which shows, although not statistically significant, that the pathogenicity of this agent and its consequences are still evident after almost 2 years of period pandemic.

ABSTRACT

Objective: Describe the epidemiology of COVID-19 deaths within a hospital in the Amazon region in a period of 64 days, which corresponds to the growth curve of the COVID-19 first-wave pandemic in 2020. Methods: The data were obtained from medical records of 152 deaths registered for adults and elderly hospitalized. The data were also compared with the number of deaths in previous years during the same period studied to assess the impact of the pandemic on this hospital. The study also assesses the impact of intra-hospital transfers, accounting for the number of times patients who died performed transfers between sectors of the hospital. Results: During the period analyzed, there was an increase in deaths compared to the previous years. The majority of dead patients were male, aged between 34 and 96 years. The deaths were associated comorbidities such as arterial hypertension, diabetes mellitus, and kidney disease. The SARS-CoV-2 infection was confirmed in 91 cases. Among them, 15 individuals were admitted without conditions related to SARS-CoV-2 infection; they had a three-fold higher number of hospital transfers than those admitted with SARS-CoV-2 infection symptoms. Sixteen patients with SARS-CoV-2 infection developed respiratory symptoms just after hospitalization. The diagnostic exam for SARS-CoV-2 infection was performed on average 4 (± 6) days after the onset of symptoms and 6 (± 6) days after admission, and the average time from the onset of respiratory symptoms to death was 4 (± 6) days. Conclusions: These data suggest the high presence of hospital infection by SARS-CoV-2 in the Brazilian Amazon region, which may be related to the number of sectorial transfers, delay in confirming the diagnosis, and lack of management. We report a serious public health problem, as it demonstrates the fragility of healthcare institutions in the hospital environmen (AU).


Objetivo: Descrever a epidemiologia de mortes por COVID-19 em um hospital na região da Amazônia em um período de 64 dias, que corresponde à curva de crescimento da primeira onda da pandemia de COVID-19 em 2020. Métodos: Os dados foram obtidos de 152 óbitos registrados em prontuários de adultos e idosos hospitalizados. Os dados foram também comparados com o número de óbitos em anos anteriores, no mesmo período estudado, de forma a avaliar o impacto da pandemia neste hospital. O estudo também avalia o impacto das transferências intra-hospitalares, contabilizando o número de vezes que os pacientes que faleceram realizaram transferências entre setores do hospital. Resultados: No período analisado, houve aumento de óbitos em relação aos anos anteriores. A maioria dos pacientes mortos era do sexo masculino, com idade entre 34 e 96 anos. Os óbitos foram associados a comorbidades como hipertensão arterial, diabetes mellitus e doença renal. A infecção por SARS-CoV-2 foi confirmada em 91 casos. Entre eles, 15 indivíduos foram internados sem condições relacionadas à infecção por SARS-CoV-2; eles tiveram um número três vezes maior de transferências hospitalares do que aqueles admitidos com sintomas de infecção por SARS-CoV-2. Dezesseis pacientes com infecção por SARS-CoV-2 desenvolveram sintomas respiratórios logo após a hospitalização. O exame diagnóstico para infecção por SARS-CoV-2 foi realizado em média 4 (± 6) dias após o início dos sintomas e 6 (± 6) dias após a admissão, e o tempo médio do início dos sintomas respiratórios até o óbito foi de 4 ( ± 6) dias. Conclusões: Esses dados sugerem alta presença de infecção hospitalar por SARS-CoV-2 na região amazônica brasileira, o que pode estar relacionado ao número de transferências setoriais, demora na confirmação do diagnóstico e falta de manejo. Relatamos um grave problema de saúde pública, pois demonstra a fragilidade das instituições de saúde no ambiente hospitalar (AU).

ABSTRACT

The study aimed to investigate the effect of anemoside B4(B4) on fatty acid metabolism in mice with colitis-associated cancer(CAC). The CAC model was established by azoxymethane(AOM)/dextran sodium sulfate(DSS) in mice. Mice were randomly divided into a normal group, a model group, and low-, medium-, and high-dose anemoside B4 groups. After the experiment, the length of the mouse colon and the size of the tumor were measured, and the pathological alterations in the mouse colon were observed using hematoxylin-eosin(HE) staining. The slices of the colon tumor were obtained for spatial metabolome analysis to analyze the distribution of fatty acid metabolism-related substances in the tumor. The mRNA levels of SREBP-1, FAS, ACCα, SCD-1, PPARα, ACOX, UCP-2, and CPT-1 were determined by real-time quantitative PCR(RT-qPCR). The results revealed that the model group showed decreased body weight(P<0.05) and colon length(P<0.001), increased number of tumors, and increased pathological score(P<0.01). Spatial metabolome analysis revealed that the content of fatty acids and their derivatives, carnitine, and phospholipid in the colon tumor was increased. RT-qPCR results indicated that fatty acid de novo synthesis and β-oxidation-related genes, such as SREBP-1, FASN, ACCα, SCD-1, ACOX, UCP-2, and CPT-1 mRNA expression levels increased considerably(P<0.05, P<0.001). After anemoside B4 administration, the colon length increased(P<0.01), and the number of tumors decreased in the high-dose anemoside B4 group(P<0.05). Additionally, spatial metabolome analysis showed that anemoside B4 could decrease the content of fatty acids and their derivatives, carnitine, and phospholipids in colon tumors. Meanwhile, anemoside B4 could also down-regulate the expression of FASN, ACCα, SCD-1, PPARα, ACOX, UCP-2, and CPT-1 in the colon(P<0.05, P<0.01, P<0.001). The findings of this study show that anemoside B4 may inhibit CAC via regulating fatty acid metabolism reprogramming.

Novel coronavirus (nCoV) namely "SARS-CoV-2" is being found responsible for current PANDEMIC commenced from Wuhan (China) since December 2019 and has been described with epidemiological linkage to China in about 221 countries and territories until now. In this study we have characterized the genetic lineage of SARS-CoV-2 and report the recombination within the genus and subgenus of coronaviruses. Phylogenetic relationship of thirty nine coronaviruses belonging to its four genera and five subgenera was analyzed by using the Neighbor-joining method using MEGA 6.0. Phylogenetic trees of full length genome, various proteins (spike, envelope, membrane and nucleocapsid) nucleotide sequences were constructed separately. Putative recombination was probed via RDP4. Our analysis describes that the "SARS-CoV-2" although shows great similarity to Bat-SARS-CoVs sequences through whole genome (giving sequence similarity 89%), exhibits conflicting grouping with the Bat-SARS-like coronavirus sequences (MG772933 and MG772934). Furthermore, seven recombination events were observed in SARS-CoV-2 (NC_045512) by RDP4. But not a single recombination event fulfills the high level of certainty. Recombination mostly housed in spike protein genes than rest of the genome indicating breakpoint cluster arises beyond the 95% and 99% breakpoint density intervals. Genetic similarity levels observed among "SARS-CoV-2" and Bat-SARS-CoVs advocated that the latter did not exhibit the specific variant that cause outbreak in humans, proposing a suggestion that "SARS-CoV-2" has originated possibly from bats. These genomic features and their probable association with virus characteristics along with virulence in humans require further consideration.

ABSTRACT

Resumo Este artigo analisa a formação internacional de Maria Palmira Macedo Tito de Morais, como bolsista da Fundação Rockefeller no campo da enfermagem, durante o Estado Novo português. Compreende-se de que forma os contextos locais influenciaram as ações da agência filantrópica internacional, o que culminou em disputas com a Organização Mundial da Saúde pela presença da enfermeira portuguesa em seus quadros. São utilizados como fonte dois dossiês sobre Maria Tito de Morais e seus dois cartões de bolsista, coletados no Rockefeller Archive Center, um relatório da Direção Geral de Saúde de Portugal e o periódico A Tribuna, consultado na Hemeroteca Digital brasileira. Conclui-se que as estratégias utilizadas pela Fundação Rockefeller, ao financiar a formação de Maria Tito de Morais com o intuito de cooptá-la para seu staff, não garantiram o controle sobre sua trajetória profissional.


Abstract The article analyses Maria Palmira Macedo Tito de Morais' international nursing education as a Rockefeller Foundation fellow, during the Portuguese Estado Novo. It studies the local contexts influence on the international philanthropic agency's actions, culminating in disputes with World Health Organization over the Portuguese nurse as staff. The sources are two dossiers on Maria Tito de Morais and her two fellowship cards collected at the Rockefeller Archive Center, a report of the Directorate-General of Health of Portugal and the journal A Tribuna, consulted at the Brazilian Digital Library. La pandemia derivada de la enfermedad por coronavirus 2019, está caracterizada por una elevada propagación en la población y la no percepción del riesgo es una amenaza que se enfrenta en la gestión de esta pandemia. Se realizó un estudio descriptivo transversal en 320 gestantes que acudieron a la consulta de ultrasonido en el Policlínico Carlos J. Finlay de Santiago de Cuba con el objetivo de realizar un diagnóstico de la percepción que tienen las gestantes sobre la COVID 19 ante la nueva normalidad. Se utilizaron variables cualitativas que evaluaron la percepción de riesgo sobre la COVID 19. Para la obtención de la información se les aplicó una encuesta. A pesar de que el 93,1 % de las encuestadas consideran la COVID 19 como una enfermedad grave, un porciento considerable saludaría a las personas con besos y abrazos, dudaría en ingresar si fuese contacto de un paciente sospechoso y confirmado, no consideraría necesario las acciones de control al viajero, realizarían fiestas familiares, no se preocuparían ante cualquier síntoma si no han tenido contacto con casos sospechosos y confirmados, consideran que la COVID no tiene consecuencias para todos, solo para los pacientes vulnerables y lo más alarmante que el 71,5 % no sienten temor a contagiarse porque las gestantes no son vulnerables. La percepción real del riesgo para la salud aún es mínima, y las gestantes no concientizan que constituyen un grupo vulnerable para esta enfermedad.


The pandemic derived from coronavirus disease 2019 is characterized by a high spread in the population and the non-perception of risk is a threat faced in the management of this pandemic. A cross-sectional descriptive study was carried out in 320 pregnant women who attended the ultrasound consultation at the Carlos J. Finlay Polyclinic in Santiago de Cuba with the aim of making a diagnosis of the perception that pregnant women have about COVID 19 in the face of the new normality. Qualitative variables were used to evaluate the perception of risk on COVID 19. To obtain the information, a survey was applied. Despite the fact that 93.1% of those surveyed consider COVID 19 to be a serious illness, a considerable percentage would greet people with kisses and hugs, would hesitate to enter if they were the contact of a suspected and confirmed patient, would not consider necessary the traveler control actions, they would hold family parties, they would not worry about any symptoms if they have not had contact with suspected and confirmed cases, they consider that COVID does not have consequences for everyone, only for vulnerable patients and the most alarming thing is that 71, 5% are not afraid of getting infected because pregnant women are not vulnerable. The real perception of health risk is still minimal, and pregnant women are not aware that they constitute a vulnerable group for this disease.


A pandemia derivada da doença do coronavírus 2019, caracteriza-se por uma elevada propagação na população e a não perceção de risco é uma ameaça enfrentada na gestão desta pandemia. Foi realizado um estudo transversal descritivo em 320 gestantes que compareceram à consulta de ultrassom na Policlínica Carlos J. Finlay, em Santiago de Cuba, com o objetivo de fazer um diagnóstico da percepção que as gestantes têm sobre a COVID 19 diante da nova normalidade. Variáveis qualitativas foram utilizadas para avaliar a percepção de risco sobre a COVID 19. Para a obtenção das informações, aplicou-se um levantamento. Apesar de 93,1% dos entrevistados considerarem a COVID 19 como uma doença grave, uma porcentagem considerável cumprimentaria as pessoas com beijos e abraços, hesitaria em entrar se fosse um contato de um paciente suspeito e confirmado, não consideraria necessário controlar o viajante, realizaria festas familiares, não se preocuparia com nenhum sintoma se não tivesse tido contato com casos suspeitos e confirmados, consideram que a COVID não tem consequências para todos, apenas para os doentes vulneráveis e o mais alarmante é que 71,5% não têm medo de se infectarem porque as grávidas não são vulneráveis. A percepção real do risco à saúde ainda é mínima, e as gestantes não percebem que constituem um grupo vulnerável para essa doença.

ABSTRACT

Design of structurally-novel drug molecules with deep learning can overcome the technical bottleneck of classical computer-aided drug design. It has become the frontier of new technique research on drug design, and has shown great potential in drug research and development practice. This review starts from the basic principles of deep learning-driven de novo drug design, goes on with the brief introduction to deep molecular generation techniques as well as computational tools and the analysis on representative successful cases, and eventually provides our perspective for future direction and application prospect about this technique. This review will provide ideas on new technique research and references for new drug research and development practice to which this technique is applied.

ABSTRACT

Objective:To analyze the clinicopathological features of de novo early colorectal cancer and to evaluate the efficacy of endoscopic treatment.Methods:Patients with de novo early colorectal cancer who underwent endoscopic resection in Beijing Friendship Hospital, Capital Medical University from June 2020 to May 2022 were enrolled. The baseline data, endoscopic manifestations, treatment methods, postoperative pathological results and prognosis of the patients were collected retrospectively.Results:A total of 33 patients with de novo early colorectal cancer were enrolled with the age of 62.67 ± 8.62 years, and the male to female ratio was 7.25∶1. The long diameter of lesions was 0.96 ± 0.36 cm. The lesion morphology was mainly superficial phenotype (type 0-Ⅱ), accounting for 72.7% (24/33). Endoscopic submucosal dissection (ESD) was performed in 29 cases and endoscopic mucosal resection (EMR) was performed in 4 cases. Postoperative pathology showed that 11 cases (33.3%) were well differentiated tubular adenocarcinoma, of which the superficial submucosal layer was invaded in 2 cases. Twenty cases (60.6%) were moderately differentiated tubular adenocarcinoma, of which the superficial submucosa layer was invaded in 5 cases and the deep submucosa layer in 15 cases. Two cases (6.1%) were moderately-poorly differentiated tubular adenocarcinoma, where the deep submucosa layer was invaded in both. There was significant correlation between the depth of invasion and the degree of differentiation ( P<0.001), and moderately and moderately-poorly differentiated lesions were more likely to invade the deep submucosa layer. The en bloc resection rate was 100.0% (33/33), the complete resection rate was 97.0% (32/33), and the curative resection rate was 42.4% (14/33). Among the 19 patients who did not achieve curative resection, 13 patients received supplementary surgical treatment. No tumor residue or lymph node metastasis was found in the postoperative pathology. All patients were followed up for 3-25 months, and no signs of local recurrence or metastasis were found. Conclusion:Most de novo early colorectal cancers are superficial phenotype under endoscopy. The pathology is mainly moderately differentiated tubular adenocarcinoma. Endoscopic resection of de novo early colorectal cancer shows encouraging short-term efficacy.

ABSTRACT

Objective:To summarize the clinical phenotype and CUX2 gene variation characteristics of developmental epileptic encephalopathy type 67 confirmed by whole exome sequencing. Methods:Clinical data of 1 case diagnosed as CUX2 gene mutations related developmental epileptic encephalopathy type 67 in the Children′s Hospital Affiliated to Zhengzhou University in January 2021 were collected, the patient′s clinical characteristics, genetic testing, head imaging, electroencephalogram results and treatment were summarized, and the patient was regularly followed-up every 3 months. At the same time, the domestic and foreign literatures on epileptic encephalopathy caused by CUX2 gene mutation were reviewed. Results:The proband was a 6 years and 4 months old girl. The main clinical manifestations included focal origin progression to bilateral tonic-clonic seizures, retardation of intellectual, language, and motor development, autistic behavior, hyperactivity disorder, and involuntary hand clapping. The video electroencephalogram showed extensive spiny slow wave and multi-spiny slow wave emission in waking and sleeping stages, and spiny slow wave and spiky slow wave emission in bilateral anterior head in sleeping stage. Brain magnetic resonance imaging (MRI) plain scan and T 2-fluid attenuated inversion recovery (T 2-FLAIR) thin layer scan showed that the signal of the left hippocampus was higher than that of the right, and the left hippocampus was slightly swollen. One month later, the brain MRI and T 2-FLAIR were reexamined. The left hippocampal signal was still slightly higher and decreased, and the hippocampal volume was slightly reduced. Whole exome sequencing showed the CUX2 gene with c.1768G>A(p.Glu590Lys) heterozygous missense variant, which was a reported de novo pathogenic variant and both of her parents were wild-type. A total of 10 cases of new heterozygous missense variants in CUX2 gene [c.1768G>A (p.Gelu590Lys)] were reported in 4 literatures. No relevant cases have been reported in China. Conclusions:Developmental epileptic encephalopathy type 67 is relatively rare. The main clinical features are seizures, global developmental delay, movement disorders, athetosis, autism and hyperactivity disorder. The heterozygous missense variant c.1768G>A (Glu590Lys) of CUX2 gene maybe the genetic cause of this case.

Resumen (analítico) Este artículo presenta una revisión narrativa de la producción empírica y conceptual en torno a adolescentes y jóvenes en orfandad a causa del homicidio (por feminicidio o desaparición de un progenitor) en América Latina. El objetivo es describir los temas emergentes del fenómeno, los cuales se sistematizan en siete ejes: 1) alteraciones e impacto psicológico; 2) secreto familiar; 3) duelo complicado; 4) estudios sobre la vivencia de la pérdida y la reconstrucción de sí mismo; 5) reestructuración familiar; 6) vulnerabilidad individual y estigmatización; 7) vulneración de los derechos humanos. La investigación con adolescentes y jóvenes es limitada; sin embargo, supone beneficios a ellos y sus familias.


Abstract (analytical) This paper presents a narrative review of the empirical and conceptual production regarding adolescents and young people from Latin America who have been orphaned due to homicide, femicide or the forced disappearance of a parent. The objective is to describe the issues that emerge from this phenomenon. A total of 42 studies that used qualitative approaches were included in this literature review. The findings were systematized in seven thematic areas: 1) alterations, psychological impact and implications for mental health; 2) family secrets and silencing of the incident; 3) complicated grief and chronic suffering; 4) studies on the experiences of loss and the reconstruction of oneself and searching for their own identity; 5) restructuring of families and new roles; 6) individual vulnerability and stigmatization; and 7) violations of human rights. Direct research with adolescents and young people is limited, however when they are included in studies it benefits them and their families.


Resumo (analítico) Este artigo apresenta uma narrativa revisada de uma produção empírica e conceitual sobre adolescentes e jovens órfãos por homicídio (feminicídio ou desaparecimento de um dos pais) na América Latina. O objetivo é descrever temas emergentes sobre esse fenômeno, que foram sistematizados em sete eixos temáticos: 1) alterações e impacto psicológico, 2) segredo familiar, 3) luto complicado, 4) estudos sobre a experiência de perda e reconstrução do self, 5) reestruturação familiar, 6) vulnerabilidade e estigmatização individual e 7) Violação de direitos humanos. A pesquisa com adolescentes e adultos jovens é limitada; no entanto, traz benefícios para eles e suas famílias.

ABSTRACT

Como Fox e Alldred (2020) consideram, o dualismo Cultura / Natureza forneceu aos filósofos, cientistas e cientistas sociais pós-iluministas uma maneira elegante de estabelecer limites para as respectivas preocupações das ciências sociais e naturais (ver também Barad, 2007; Braidotti, 2013; Fullagar et al., 2019). Este dualismo tem permitido a criação de distinções entre corpos e modos de estar-no-mundo "modernos" (leia-se "civilizados") e "tradicionais" (leia-se "primitivos") (Denowski e Viveiros de Castro, 2014). No entanto, ao explorar criticamente as questões de incorporação (embodiment), a influência do ambiente construído sobre o bem-estar, as transições climáticas e/ou a pandemia de Covid-19 em curso, tais distinções começam a se tornar problemáticas, como argumentado eloquentemente nas últimas três décadas por debates e proposições feministas, pós humanistas, novo-materialistas e ecológico politicos, entre outros. Dando continuidade a um diálogo contínuo iniciado em 2018 entre acadêmicos e ativistas da América Latina e da Europa (ver Donato, Tonelli, Galak, 2019), este seminário explorou como os domínios inter-relacionados de saúde, atividade física e educação podem ser a partir de perspectivas que de des-estabilizar fronteiras ontológicas estabelecidas entre natureza, cultura, corpo e sua relação. Isto foi feito através de um diálogo entre Alessandro Bortolotti, Simone Fullagar, BrunoMora, Niamh Ni Shuilleabhain, (Austrália, Itália, Reino Unido e Uruguai, respectivamente). O evento online ocorreu como o primeiro de uma série de seminários online de duas partes sobre Remontando o nexo natureza-cultura-corpo: práticas e epistemologias.

ABSTRACT

Chordomas are rare locally invasive malignant bone tumors arising from remnants of embryonic notochord. Dedifferentiated chordoma (DC), a rare subtype, is characterized by the presence of a sarcomatous component in conventional chordoma (CC) which may arise de novo or as a malignant transformation of previously treated chordoma. The presence of dedifferentiation warrants a poor prognosis due to distant metastasis and recurrences. De novo DCs pose a diagnostic challenge especially in small biopsies and at metastatic sites. Here, we report the case of a 45-year-old female presenting with a long history of backache and constipation, finally diagnosed as DC. Radiological as well as histomorphological pictures of the tumor posed diagnostic challenges because they can mimic other tumors occurring in a similar location. We found this case worth reporting as de novo DC is rarely reported in the literature and it has the potential to pose diagnostic as well as therapeutic challenges.

Resumo: Este trabalho indaga o caráter avaliativo do Programa Nacional de Melhoria do Acesso e Qualidade da Atenção Básica na Atenção Primaria à Saúde no Brasil, a partir do caráter performativo deste instrumento no cenário mundial de crescente managerialização das políticas sanitárias. Objetivo: Compreender como a qualidade do acesso se define em termos práticos e quais ações são promovidas aos trabalhadores da Atenção Primaria à Saúde através da Portaria 1.645 e o Instrumento de Avaliação aplicado aos profissionais do Núcleo Ampliado de Saúde da Família e Atenção Básica. Métodos: Foi realizada uma Análise Pragmática do Discurso, com auxílio dos Softwares Atlas.ti 8 e Microsoft Excel 2010. Resultados: Os resultados revelam estândares e indicadores promovidos pelo Programa Nacional de Melhoria do Acesso e Qualidade da Atenção Básica, assim como o papel ambivalente de gestores e profissionais da saúde, entre seu caráter voluntário e proativo e, revelam o desmonte do modelo de Atenção Primária à Saúde sob lógica comunitária no Brasil, em comparação à abordagem do Novo Management Público e a atuação em função de indicadores. Conclusões: O papel do Estado na garantia de direito a saúde é minimizado e dá lugar ao Novo Management Público na saúde.

Objetivo: Comprender cómo se define la calidad del acceso en términos prácticos y qué acciones se promueven a los trabajadores de Atención Primaria de Salud a través de la Ordenanza 1.645 y el Instrumento de Evaluación aplicado a los profesionales de Núcleo Ampliado de Saúde da Família e Atenção Básica. Métodos: Se realizó un Análisis Pragmático del Discurso, con la ayuda de Softwares Atlas.ti 8 y Microsoft Excel 2010. Resultados: Los resultados revelan estándares e indicadores promovidos por el Programa Nacional para la Mejora del Acceso y la Calidad de la Atención Primaria, así como el papel ambivalente de los gerentes y profesionales de la salud, entre su carácter voluntario y proactivo y revelan el desmantelamiento del modelo de Atención Primaria de Salud bajo lógica comunitaria en Brasil, en comparación con el enfoque del Nuevo Management Público y el desempeño en función de los indicadores. Abstract: This paper questions the evaluative process of the National Program for Improving Access and Quality of Primary Care in Primary Health Care in Brazil, based on the performative character of this instrument in the world scenario of increasing management of health policies. Objective: To understand how the quality of access is defined in practical terms and what actions are promoted to Primary Health Care workers through Ordinance 1,645 and the Evaluation Instrument applied to Extended Centers for Family Health and Primary Care professionals. Methods: A Pragmatic Discourse Analysis was performed, with the help of Atlas.ti 8 and Microsoft Excel 2010 Software. Results: The results reveal standards and indicators promoted by the National Program for Improving Access and Quality of Primary Care, as well as the ambivalent role of managers and health professionals, between their voluntary and proactive character and reveal the dismantling of the Primary Health Care model under community logic in Brazil, compared to the New Public Management approach and the performance in function of indicators. Conclusions: The role of the State in guaranteeing the right to health is minimized and gives way to New Public Management in healthcare.

Histoid leprosy, an unusual variant of lepromatous leprosy, originally thought to manifest following dapsone monotherapy, is being increasingly reported ‘de-novo’. We hereby report four de-novo cases; one mimicking lupus pernio presenting with an uncommon lesion (plaque) at an extremely rare site (nose). Of the remaining three classical cases, one exhibited a not-so-frequent development of erythema nodosum leprosum (ENL) reaction that needed modified multidrug therapy for two years, other three responded well to standard multibacillary multidrug therapy which was also given for two years.

ABSTRACT

The reprogramming of lipid metabolism and signaling pathways is the central aspect of cancer biology. It is hypothesized that tumor cells can alter the lipid spectrum in order to fulfill their metabolic requirements. Furthermore, they can alter potential tumors and suppressive mechanisms in which lipids' involvement is essential. Recently, more attentions have been given on the alteration of lipid metabolism during prostate cancer development, and investigations have shown unique regulation of "de novo" lipid synthesis in cancer cells. Cancer cells often use newer pathways and enzymes to simplify the synthesis of fatty acids, and the newly synthesized lipids affect cellular processes, which impacts cancer cell proliferation and survival outcomes. Herein, we aimed to study the influence of lipid profile alterations on the development of prostate cancer. We found that the total amounts of lipids and phospholipids were increased within tissues from men with the malignant prostate tumor as compared with the benign prostate tissue. Significant changes were also observed in the composition of saturated and unsaturated fatty acids within the malignant tumor tissues. Intensification of lipid peroxidation has also been observed in malignant prostate tumors compared to benign prostate tumors. Collectively, these findings further highlights the fact that lipid and fatty acids play unique regulatory roles in the cellular development of prostate malignant transformation.

Introducción: El término insuficiencia cardíaca de novo hace referencia a pacientes sin diagnóstico previo de dicha enfermedad. La revisión de este tema deviene de un problema real, pues muchos pacientes acuden a la atención secundaria sin diagnóstico previo de insuficiencia cardíaca y además en estadios avanzados. Objetivo: Describir los elementos claves para el diagnóstico precoz de insuficiencia Cardíacas. Métodos: Se llevó a cabo una búsqueda bibliográfica en las siguientes bases de datos: Pubmed, SciELO, ESBCO, Cochrane y BVS, así como en diferentes webs médicas durante tres meses entre febrero de 2021 al 31 de mayo de 2021. Conclusiones: El diagnóstico precoz de insuficiencia cardíaca permitió disminuir el número de pacientes que llegan a la Atención Secundaria sin diagnóstico previo. Existen clasificaciones que identifican estadios tempranos de la enfermedad y la de la ACC/AHA es relevante para lograr este objetivo. En atención primaria esto es un reto si no se emplean pruebas diagnósticas como el ecocardiograma. Es importante la determinación de las concentraciones circulantes del péptido natriurético tipo B (BNP) y del fragmento N-terminal de su protohormona (N-terminal BNP). Este biomarcador debería estar accesible en las consultas ambulatorias para pacientes que presentan sospecha clínica de insuficiencia cardíaca «de novo»(AU)


Introduction: The term di novo heart failure refers to patients without a previous diagnosis of this disease. The review of this issue comes from a real problem, since a group of patients attend secondary care without a previous diagnosis of heart failure and also have in advanced stages. Objective: The objective is to provide a clue that facilitates the early diagnosis of heart failure. Methods: A bibliographic search was carried out in the following databases: Pubmed, SciELO, ESBCO, Cochrane and BVS, as well as in different medical websites for three months (February 1, 2021 to May 31, 2021). Conclusions: The early diagnosis of heart failure will allow us to reduce the number of patients who arrive at Secondary Care without a previous diagnosis. There are classifications that identify early stages of the disease, being in our opinion the ACC / AHA classification the one that should carry the most weight. In primary care this can be a challenge if diagnostic tests such as echocardiography are not used. Dosification of serum levels of type B natriuretic peptide (BNP) and the N-terminal fragment of its protohormone (N-terminal BNP) is very useful. This biomarker should be accessible in outpatient clinics for patients with clinical suspicion of di novo heart failure(AU)

ABSTRACT

Homem de 53 anos, hipertenso e portador de bronquite, admitido em um serviço de urgência no dia 15 de dezembro de 2020 devido sintomas gripais, febre e cefaleia iniciados há oito dias. Após constatação de acometimento pulmonar importante mediante tomografia computadorizada (TC) de tórax, sugestivo de infecção pelo vírus SARS-CoV2, o paciente foi internado em unidade de terapia intensiva. Foi intubado no décimo dia de internação, e, dois dias após, evoluiu com labilidade pressórica importante, recorrendo ao uso de noradrenalina e nitroprussiato, além de outros anti-hipertensivos, conforme a necessidade. O quadro predominante foi a hipertensão arterial sistêmica, manifestada principalmente com a mudança de decúbito, sendo o maior valor pressórico registrado de 240x90 mmHg. A disautonomia também se manifestou por ausência de dejeções, sudorese excessiva e espasmos musculares. A frequência cardíaca se manteve estável e dentro dos parâmetros de normalidade.A partir do trigésimo dia de internação, observou-se melhora progressiva do quadro e reestabelecimento da homeostase. Obteve alta após 59 dias de internação, sem sequelas significativas. A explicação mais razoável para o caso é o aumento da resistência vascular periférica, por ação da angiotensina II, associada à supressão do sistema parassimpático, o que explica, também, a incompetência do barorreflexo para compensação da frequência cardíaca. Adicionalmente, o paciente estava em uso de carvedilol. Este caso enfatiza o desafio diagnóstico precoce da disautonomia em pacientes críticos, devido a carência de ferramentas adequadas para uso na prática cotidiana. A estimulação vagal pode constituir opção terapêutica eficaz, mas carece de mais estudos


A 53-year-old male, hypertensive and with bronchitis, was admitted to the emergency department on December 15, 2020 due to flu-like symptoms, fever and headache that started eight days ago. After finding significant lung involvement by chest computed tomography (CT) suggestive of SARS-CoV2 virus infection, the patient was admitted to the intensive care unit. He was intubated on the tenth day of hospitalization, and, 2 days later, he evolved with significant pressure lability, using norepinephrine and nitroprusside, in addition to other antihypertensive drugs, as needed. The predominant state was hypertension, expressed mainly when there is interference from the patient's position in bed. The highest pressure value recorded was 240x90 mmHg. Dysautonomy was also manifested by the absence of stools, excessive sweating and muscle spasms. Heart rate remains stable and within normal limits. From the thirtieth day of hospitalization onwards, there was an evolution with progressive improvement and restoration of homeostasis. He was discharged after 59 days of hospitalization, without sequelae. The most reasonable explanation for the case is the increase in peripheral vascular resistance, due to the action of angiotensin II, associated with the suppression of the parasympathetic system, which also explains the incompetence of the baroreflex to compensate the heart rate. Additionally, the patient was using carvedilol. This case emphasizes the importance of tools that early identify dysautonomy, prepare the team. Vagal stimulation can be an effective therapeutic option, but further studies are needed
